This training module explores geriatric care models, focusing on interdisciplinary approaches in providing home-based primary care, interprofessional team practices for older adults, and community complex care response teams. Emphasis is placed on the 4M's framework, challenges in care coordination, and evidence-based outcomes for improving quality of life in older populations.
