The Coverage to Care (C2C) initiative is designed to help individuals understand and effectively use their health coverage to access necessary healthcare services. This initiative primarily targets vulnerable populations, including low-income individuals and minority communities, to bridge gaps in healthcare access and awareness. Community Health Workers (CHWs) play a vital role in educating individuals about health insurance, connecting them to preventive services, and guiding them through the healthcare system to ensure they receive the care they need.
